The idea
Populations across the US, Europe, China and Japan are aging fast, and I think that drives structurally rising demand for drugs treating diseases of old age: Alzheimer's, heart failure, kidney disease, cancer. I split the portfolio evenly between drugmakers with deep pipelines in neurodegeneration, cardiometabolic disease and oncology, and the suppliers, contract manufacturers, bioprocessing tools and trial-runners, who get paid regardless of which drug wins. No single name dominates either side, and I keep a modest cash reserve since this is meant to play out over years, not quarters.
What it owns
- Contract manufacturers, trial runners and injectable packagers collect fees based on drug volume, earning regardless of which specific drug succeeds. · Thermo Fisher ScientificIt manufactures, tests and runs trials for drugs across the industry, earning on development and manufacturing volume independent of which drugmaker wins., West Pharmaceutical ServicesIt manufactures the stoppers and injection components for a dominant share of injectable drugs, collecting a fee on every vial of aging-disease biologics sold., Charles River LaboratoriesIt runs the preclinical safety testing nearly all new drugs pass through before human trials, earning on the industry's entire aging-disease pipeline., Icon plcIt conducts clinical trials for drugmakers, and trials in Alzheimer's, heart failure and cancer represent exactly where aging-driven pipelines concentrate spending., Stevanato GroupIt manufactures the vials, syringes and pen cartridges that deliver injectable aging-disease drugs, collecting a fee on container volume as drug volumes grow.23%
- Equipment and consumables required to manufacture every biologic drug function as a toll on the entire development and production pipeline. · DanaherIt supplies the filtration, media and equipment manufacturers use to produce biologic drugs, collecting a toll on production volume regardless of the specific medicine., Agilent TechnologiesIt sells lab instruments and testing supplies the pharmaceutical industry uses to develop and verify drugs, spending that scales with industry size rather than individual pipelines., Waters CorporationIt manufactures instruments that verify drug identity and purity on every batch, a regulatory requirement that creates a toll on all drug production volume., RepligenIt supplies filtration and purification equipment used in biologic drug manufacturing, earning on production batches regardless of the specific medicine.23%
- Cancer incidence rises steeply with age, concentrating the patient base for modern cancer and immunologic drugs almost entirely in elderly populations. · MerckIts Keytruda cancer drug dominates the world's largest cancer franchise, a disease overwhelmingly concentrated in the elderly., AbbVieIts inflammatory disease drugs Skyrizi and Rinvoq target conditions that accumulate with age and generate cash to fund the next wave of aging-disease therapies., RegeneronIts Eylea treats age-related macular degeneration, an eye disease that strikes almost exclusively the elderly, plus a growing cancer and immunology pipeline., Johnson & JohnsonIts large, diversified pharma portfolio spans cancer, immunology and neuroscience drugs sold predominantly to older patients across developed markets.19%
- Heart failure, diabetes and kidney disease rank among the largest pharmaceutical markets in aging populations and drive the bulk of spending on chronic disease. · Novo NordiskIts diabetes, obesity, heart and kidney drugs treat the primary diseases of aging bodies and depend on rising demand from an older global population., AstraZenecaIts kidney and heart failure drugs, led by Farxiga, directly treat two defining diseases of old age from aging populations around the world., Bristol-Myers SquibbIts cancer, heart and blood disorder drugs target older patients but trade at a discount because the market questions the pipeline replacing expiring patents.16%
- Alzheimer's and related brain diseases grow in lockstep with populations over seventy-five, and disease-modifying treatments are entering the market for the first time. · Eli LillyIts Alzheimer's treatment and cardiometabolic drugs directly address the two disease classes that expand as populations age., BiogenIts Leqembi Alzheimer's treatment is rolling out across the aging developed world and depends on growing uptake from the elderly population.11%
